Thank you for your interest in the Colorado Agricultural Addiction and Mental Health Program.

Current Provider

Provider Expectations

• Our contracted rate is $90/hour. We ask that you continue to honor this rate for self-pay if the individual continues to pursue care beyond the six voucher availability.

• We ask that you complete a three hour cultural competencies course about agriculture and rural communities. This required training is participatory and virtual unless otherwise specified.

• We ask that you are open to new clients with some scheduling availability and that, if at any point you are no longer accepting new patients, you contact us immediately to permanently or temporarily remove you from the available providers list.

• This is not an assurance of referrals or client participation.

• For reimbursement, we will require you to provide basic demographic and diagnosis information with no personal, identifiable information.
